ABSTRACT

INTRODUCTION: Perforation of tympanic membrane constitutes a major portion of patients attending ENT OPD for which they are advised surgical procedures. Most of the cases with small perforations can be managed by doing OPD procedure of cautery patching and hence surgical procedure can be avoided. METHODS: The study was conducted in the tertiary hospital from August 2018 to July 2019. The patients were selected on the basis of the inclusion criteria after doing clinical examination, and audiometry. All the patients underwent cautery patching with tincture ferri perchloride and patch placed. The patients were followed up weekly till the perforation was completely closed or uptill 3 months. RESULTS: Total of 65 patients was included in the study. Most common etiology of perforation was inflammatory involving anteroinferior quadrant as most common etiology with success achieved in 56 patients. CONCLUSION: patients with small central dry perforation of tympanic membrane, cautery patching using tincture ferri per chloride and paper patching gives good results comparing to the surgical procedure, while ablating the morbidity and psychological trauma of the surgery.

ABSTRACT

Rice germplasms collected from different regions could be used as valuable resources for the future breeding programme. For the utilization of such collections, knowledge about the level and distribution of genetic diversity among these collections will facilitate the breeder. In this study, we report the phenotypic correlation, biochemical quality parameters and population genetic analysis of 35 rice accessions including 34 aromatic rice from different countries and a nonaromatic, Nagina 22, a well-known drought resistance variety. Further biochemical quality analysis, gel consistency test, molecular diversity analysis with 55 simple sequence repeat markers, population structure analysis and pair wise FST analysis were also conducted to assess the genetic diversity. The collected rice genotypes showed significant variability in different agronomic traits, i.e. spikelet per panicle, branches per panicle etc. Results obtained from the above tests demonstrated the importance of regional genetic studies for understanding the diversification of aromatic rice in Asian and African rice.

ABSTRACT

Background: A comparative prevalence studies using modified WHO and more stringent criteria of WHF2012 have not been conducted in this part of the country and so authors felt necessary to conduct such study in this North-eastern state of India.Methods: In this cross-sectional study, 3600 children aged 5-15 years (11.07±2.86, 51.90% male) from randomly selected schools of Manipur were examined clinically and by 2D colour doppler echocardiography.Results: Only one case of Clinical RHD with a combination of MR and AS was found, (prevalence of 0.28/1000) [CI: 0.04-1.97). Echocardiography detected 3 cases of definite subclinical (prevalence rate of 0.83/1000 (CI: 0.27-2.58], 14 cases of borderline subclinical RHD (prevalence rate of 3.9)/1000 (CI: 2.30-6.56) befitting WHF 2012 criteria, and 61 cases of borderline subclinical RHD befitting modified WHO criteria (prevalence rate of 16.9/1000)[ CI1.3-2.1].On follow up to 5 years, among the 3 cases with definite subclinical RHD, 2 had worsened and 1 remained non- progressive, while among the 14 subclinical borderline cases, 2 normalized, 6 remained non-progressive and 6 children were lost to follow up.Conclusions: Prevalence of RHD using echocardiography is several folds higher compared to clinical examination alone. While the number is reduced by about half using WHF criteria. However, the important of WHO criteria cannot simply be neglected, as all the subclinical cases by WHF criteria remains non-progressive, while one case of subclinical defined by WHO worsen on follow up.

ABSTRACT

Primary progressive aphasia (PPA) is thought to be a disorder of focal cortical degeneration which occurs as a result of lobar atrophy of dominant frontal and temporal lobe. We report a case of a 78 year old male patient presenting with progressive language affection predominantly and clinically diagnosed as PPA but magnetic resonance imaging (MRI) brain showed an unusual finding of reverse 'hot cross bun' sign in pons in T2 weighted (T2W) / diffusion weighted image (DWI)/ T2 fluid attenuated inversion recovery (FLAIR) axial views. This is the first case report of reverse 'hot cross bun' sign in a case of PPAto the best of our knowledge.

ABSTRACT

The objective of the following study is to assess the effect of steroidal anti-inflammatory agent on the outcome of ureterorenoscopic lithotripsy [URSL] for ureterovesical junction [UVJ] calculus. This was a prospective randomized controlled study conducted at the Department of Urology, Regional Institute of Medical Sciences, Imphal. One hundred and twenty-six patients requiring ureteroscopic lithotripsy for UVJ calculus were randomly assigned into two groups. The study group received tablet deflazacort 30 mg once a day for 10 days prior to the procedure, whereas the control group did not receive such treatment. Parameters with respect to the outcome of the procedure were recorded for all patients in both groups. Fisher's exact and independent t-test was used to compare the outcome between the groups where P < 0.05 was considered to be statistically significant. There was significant statistical difference [P - 0.016] on the endoscopic appearance of the region of ureteric orifice in patients receiving steroidal anti-inflammatory agent compared with control. Severe procedure related pain and mean operative time was less in the study group compared to control [P - 0.020 and 0.031, respectively]. Re-treatment rates in the study group were lower than the control group [4.76% vs. 17.46%] and found to be statistically significant [P - 0.044]. It is found that computed tomography [CT] appearance [r- 0.399] and stone size [r - 0.410] strongly correlate with the endoscopic findings of the region of UVJ [P - 0.001]. Inflamed and or obliterated ureteric orifice is the major constraints for stone clearance at ureterovesical junction. The present study showed the administration of tablet deflazacort [a steroidal anti-inflammatory agent] significantly improves the outcome of URSL under local anesthesia. We strongly recommend its use prior to URSL for UVJ calculus, especially for stone size >/=10.24 mm and on CT evidence of prominent soft tissue swelling at the UVJ

ABSTRACT

To evaluate the effect of parenteral testosterone on penile length, preputial skin and side effects in patients with hypospadias. 23 patients with hypospadias were included in this study. An oily solution, each ml of which contained testosterone propionate 25 mg, and testosterone enanthate 110 mg, equivalent to 100 mg of testosterone was given deep intramuscularly 4, 3 and 2 weeks before reconstructive surgery at the dose of 2 mg/kg body weight. Increase in penile length, transverse preputial diameter, and diameter at the base of penis were noted. Basal testosterone levels were obtained before the institution of therapy and on the day of operation. In addition, side effect such as development of pubic hair and delay in bone age was noted. Following parenteral testosterone administration, the mean increase in penile length, transverse preputial diameter and diameter at the base of penis was 1.35 +/- 0.40 cm [P<0.001], 1.40 +/- 0.47 cm [P<0.001], and 0.72 +/- 0.47 cm [P<0.001], respectively. Serum testosterone level after injection was well within normal range for that age. Minimal side effects were noted in form of development of fine pubic hair. We conclude that parenteral testosterone can be safely used to improve the surgical outcome of hypospadias repair
